How To Submit A Request:
Submit a written request that reasonably describes the records you seek. You may:
- Use the online form (preferred)

Tip: Include dates, addresses, incident numbers, and any keywords to help us locate records quickly.
Please be advised that we do not maintain medical patient records, but the overall scope of the incident. To request medical records, please contact the EMS service provider for our area: Harris County Emergency Corps: https://hcec.com/
Next Steps:
- We respond within 10 business days to: provide records, give a date/time for release, or seek a ruling if an exception may apply.
- If your request is large/complex, we may send a cost estimate or a request to narrow/clarify.
- If we send a cost estimate over $40, you must reply within 10 business days or the request may be considered withdrawn.
Business days exclude weekends and official holidays. The clock starts the business day after we receive your request.
Fire Reports Information & Fees:
Inspection of records is generally free.
- Fire incident reports are public records under the Texas Public Information Act.
- Certain confidential information may be redacted (e.g., juvenile data, personal identifiers, ongoing investigations).
Fees:
- Standard physical copies: $0.10 per page
- Certified physical copy: $1.00 per document
- Postage: actual cost if mailed
- Email PDF (no charge if <15 min retrieval)
Motor Vehicle Accident (Crash) Reports Information & Fees:
- Governed by Texas Transportation Code §550.065
- Only released to authorized parties: persons involved, owners, attorneys, insurers, parents/guardians, etc.
- If you do not qualify, we cannot release the crash report, but you may request a basic incident summary under the Public Information Act.
Fees:
- Standard physical copy: $6.00
- Certified physical copy: $8.00
- Email PDF (no charge if <15 min retrieval)
MVR Fee with Affidavit & Postal Service Example:
- Motor Vehicle Report (certified copy) $8.00
- Affidavit of Records: $20.00
- Notary Fee (Affidavit notarized- 1 Signature) $10.00
- Postal Fee $0.78
- Total Estimated Fees Due: $38.78
Fees are estimates only and may vary depending on the nature, volume, or redaction requirements of the request.
Certified Copies & Affidavits:
- Affidavit of Records (optional, courtesy service): $20.00 administrative fee
- Applicable Notary fee (if affidavit notarized):
- $10.00 for the first signature
- $1.00 for each additional signature
- $1.00 per page for certified photocopies (if requested)
Request Scope & Privacy:
- Requests must seek existing records; we are not required to create new documents or answer general questions.
- Some information may be withheld under legal exceptions (e.g., privacy, ongoing investigations). When required, we request an Attorney General ruling.
- Please do not submit PHI/SSNs unless necessary to identify records (e.g., your own medical record release).
